Key Responsibilities: • Maintain regular contact with patients and their families to ensure a high standard of care. • Complete all required monthly and annual documentation accurately and on time. • Coordinate patient transportation and medical supply orders. • Conduct quarterly in-person visits to assess patient needs and build relationships. • De-escalate situations when patients are dissatisfied. • Maintain regular office hours • Monday–Friday, 9:00 AM–5:00 PM.
Ideal Qualifications: • Associate's or Bachelor's degree required. • Strong organizational, communication, and problem-solving skills. • Proficient in basic computer skills (email, documentation, data entry). • Experience in case management or patient coordination is a plus. • Bilingual ability is a plus.
Salary: Starting at $52k–60k/Year.
